HEALTH EQUITY INITIATIVES: Advocate Health Care is partnering with the Public Health Institute of Metropolitan Chicago and the University of Illinois at Chciago to create a Center for Faith and Community Health Transformation. The Center will advance faith-led community partnerships that promote the health and well-being of communities where health inequities are prevalent.

Advocate also partners with several FQHCs to provide accessible and affordable primary care, supports a community fund in one of our service areas that provides grants to local organizations that are addressing health inequities and maintains an aggressive charity care policy.

- Use the series and materials in trainings or classes
- Post and share information about the series with members and constituents
