HEALTH EQUITY INITIATIVES: We have been healthy community advocates since 1993 and have used the determinants of health in our model for population health improvement since the late 90's. Issues of the poor and working poor are a major challenges as we proceed in our work for youth, senior, community and workplace health improvement.

In July our Foundation (Heartland Foundation) hosted a regional summit featuring "Unnatural Causes" followed by a health status reveiw including: the social determinants, lifestyle and chronic disease and the built environment. Now we are planning a follow up training for local and regional advocates to replicate the process in their community and within the scope and priorities of their interests.
